Admission Form and Application Procedure

Due to COVID-19 pandemic situation if any student wants to submit their admission FORM online, please follow the following procedure.

  1. Download the FORM from our website.
  2. Pay the Application Form Fee 1000 taka by Payment option of Nagad or Scan the following QR Code by Nagad app.
  3. Fill-up the FORM and must input the Payment information on the Form’s field #15.
  4. Attach the FORM with PP size photograph, all other necessary papers and sent to our official email address
  5. Please sent the details of your payment & attach your Filled up Form’s scan copy as PDF format.
  6. Email’s subject should be specific by mentioning Your Full Name & DGHS Roll Number.
  7. After verifying your given information we will sent you a confirmation email from our official email.
